We are excited to welcome you and your clients to our premier independent training facility! Your first session is on us! Before you get started we would like you to take a tour of our facility and meet with one of our owners. Please visit our contact page and complete our form to make an appointment. Below is a list of what we ask of Independent Trainers and new clients prior to making an appointment at Elysium.

Independent Trainer Requirements

  1. Copy of your national certification

  2. Copy of your CPR/AED certification

  3. Liability Insurance: $1 million in coverage with Elysium Fitness LLC and the address listed as additional insured

  4. A completed Personal Trainer Policies and Procedures Form

  5. A Covid-19 Liability Release Form

  6. Proof of Covid-19 vaccination

(Training fee for 1-hour is $34 per session after your first visit)

All trainers are required to complete the above three forms through the link below or by downloading and emailing the completed forms to elysiumfitness72@gmail.com prior to making your first appointment at the gym.

New Client’s Requirements

  1. Guest Waiver/Medical History Form

  2. Covid-19 Liability Release Form

  3. Proof of Covid-19 vaccination

To Book a Training session

In order to allow for contact-less booking we have moved our scheduling onto the Mindbody mobile app. You can download the app onto your Apple or Android device.

Below are instructions on how to book a session for you and your client. At this time, we are offering 1-hour and 30-minute sessions as “Appointments”.

  1. Download the Mindbody mobile app.

  2. Create an account with the same email address we have on file for you. If you would like to update your email address, please send us a request via email or on the phone before you create an account.

  3. Search for Elysium Fitness. The gym should appear on the business search results. On the gym’s page you will see our schedule.

  4. Click on "View Schedule", then on “Training Appointments” and choose between a 30 minute training reservation, or a 60 minute training reservation. You will have the choice to pay for a single session or a pack of 10 sessions when it’s time to choose a payment option later on. 

  5. You will be asked to “Pick a Staff”, please choose “Any Staff”.  The staff members you see just represent the 4 available slots for a trainer and one client. This will ensure the appropriate social distancing measures in the facility. We will only allow 4 trainers with clients on the floor at a time.

  6. Next, you will see a calendar showing what appointment times are available during the week. You will see “M” for morning times, “A” for afternoon times, and “E” for evening times. Please choose the day you are interested in to see the specific times that are available on the next screen.

  7. After choosing the correct time for you and your client, you will be taken to the “Appointment Booking” page. Please make sure to include your client’s name in the “Notes” section before selecting your payment option. 

  8. After you have reviewed the appointment details and included your client’s name in the “Notes” section, hit “View Pricing” to select a payment option. By default, the single session price will be displayed. If you would like to buy a pack of 10 sessions, just click on the payment displayed to see our other options. If you choose to buy a 10 session pack, you will see it appear as a payment option when you book your next appointment. You will be able to see the sessions under “Your Passes” on your account profile.

  9. Once you are ready to confirm your appointment, hit “Book and Buy”. If it is your first time using Mindbody, the app will ask you to add payment information. Please make sure to use the same credit card we have on file for you. If you need to add a new credit card to your file at our facility, let us know before you book appointments on the mobile app. We need to have the same information on the mobile app and on the gym’s computer to allow for refunds if needed.

  10. Once the payment is processed, you can see the upcoming appointment under “Schedule” on your account profile. You will also receive a confirmation email. Please make sure to arrive at the time of your appointment and not earlier to ensure we maintain our restricted capacity to 4 trainers with clients on the floor at a time. Also, please make sure to leave on time to allow other trainers to begin their sessions on time. Your cooperation is extremely appreciated!
